Guidance Undoing some radio modifications

Hi all,

I've got a Verizon Galaxy S5 that's been modified to behave like a 3G phone -- 4G/LTE is disabled and the ESN has been swapped with another phone that I have (disassembled) in my possession. I've used it this way for a year or so, but my carrier (Selectel) supports 4G now, and I am looking to undo the modification. The person who made this change for me can't be reached, and I can't remember the name of the software I need to do the job.
Can anyone point me in the right direction?
(I've rooted/flashed/etc. extensively and I program computers for a living, so actually doing this shouldn't be an issue once I have the tools, but my memory is failing me.)

Thank you.
